In order to make your first visit as easy as possible, please print off the above two forms and fill them out prior to your first appointment. Please print neatly in pen so we can quickly get your information transferred into the computer.

Feel free to provide the forms to us with one of these methods:

1. Just bring them completed to your appointment

2. Fill them out at home and fax them in to us at

4. For the computer savy: fill them out by hand at home, scan them in, and send them as a PDF to:
